• Raffle runs August 16 through September 30, 2021.  
• 3 winners will be selected each Friday, beginning September 3 and ending October 1, 2021.
• All Prospect Heights Library patrons are eligible to win by completing this form.  
• Valid library card number must be on file in our patron database.
• Only one entry per person.
• Winners will be contacted by phone or email.  
• In the event a winner cannot be reached within 5 days of drawing, another winner will be selected.  

To help spread the word about the value of having a library card, all winners' names (first name and last initial only) will be posted on our website and shared on our social media.
